Job summary
Main duties of the job
In your role, you will provide training, direct line management, and supervision to the IDS administration team. You will be responsible for ensuring strict compliance with Trust policies and procedures at all times.
You will offer guidance and support to the wider team across the Trust, contributing to the smooth operation of the Integrated Discharge Service and helping to ensure safe and timely patient discharges.
As part of your responsibilities, you will ensure that administration assistants receive appropriate training and ongoing development, enabling the team to consistently deliver an excellent service.
Working closely with the IDS Hub administration lead, you will support robust service delivery. This will involve effective communication and the promotion of integrated working across health, social care, and both acute and community services.
You will also play a key role in supporting national reporting, maintaining data integrity, and ensuring that all KPIs are consistently achieved or exceeded.
Using continuous improvement methodology, you will identify and implement appropriate service improvements, contributing to the ongoing enhancement of the service.

- Manage referrals and discharges from the hospital and help assist with the co-ordination of them in line with national and locally agreed standards and guidelines

Ensure queries from patients and their families are responded to in a timely and appropriate way
- To ensure that all discharges are facilitated in accordance with Trust and local policies
- To escalate any capacity or performance concerns to senior colleagues in a timely manner.
- To provide training and support to new and ongoing IDS team in various roles.
- Responsible for completion of various reports and returns in line with policy and procedure ensuring data integrity and professional standards are adhered too
Person specification
Qualification
- Educated to GCSE standard or equivalent with Grade C or above in Maths and English.
- Educated to NVQ 3 level in a relevant subject or equivalent level of qualification, or significant previous proven experience in health or social care setting
- Supervisory or Management qualification
Knowledge & Experience
- Experience of working under pressure and managing a large workload
- Knowledge of NHS performance reporting/targets
- Substantial experience working in an administrative role using IT solutions and systems.
- Experience of ability to liaise and coordinate with internal departments and external organisations
- Experience of producing high quality reports ensuring collation of all relevant information and actions
